To maintain membership in RMD Botox & Beauty Club, you must complete a Botox treatment every 90-120 days with a minimum of 40 units each visit at $10 per unit.
All appointments must be scheduled in advance with a $60 deposit which can be rolled over onto subsequent appointments. This $60 deposit will be applied towards your 4th treatment.
When you sign up, you must understand that if you fail to complete the first 3 treatments within the designated time-period, you will have to pay the regular price for your Botox. There will be NO EXCEPTIONS.

**Cannot be combined with other office specials/offers however you may use Alle Rewards benefits.
***Botox Club will be void: if you no-call/no-show after 120 days from prior treatment, if you decide to do less than 40 units during the allowed time period, if you use Groupon, or if you receive Botox as a Model for training purposes, the $60 deposit will be lost.
