To maintain membership in RMD Botox & Beauty Club, you must complete a Botox treatment every 90-120 days with a minimum of 40 units each visit at $10 per unit. Upon completion of 3 treatments, you will receive a complimentary SkinVIVE treatment OR 1 syringe of Juvederm Filler OR Laser Genesis at the time of your 4th treatment. This free treatment must be completed the day of your 4th Botox session. 
 

All appointments must be scheduled in advance with a $60 deposit which can be rolled over onto subsequent appointments. This $60 deposit will be applied towards your 4th treatment. 

When you sign up, you must understand that if you fail to complete the first 3 treatments within the designated time-period, you will have to pay the regular price for your Botox and will NOT receive the complimentary SkinVIVE/Filler/Laser Genesis at your 4th treatment and the $60 deposit will be lost. There will be NO EXCEPTIONS.
