To maintain membership in RMD Botox & Beauty Club, you must complete a Botox treatment every 90-120 days with a minimum of 40 units each visit at $10.50 per unit. All appointments must be scheduled in advance with a $100 buy-in fee before/at your first appointment. This $100 buy-in fee locks you in at $10.50 per unit and the additional benefits. When you sign up, you must understand that if you fail to complete the 4 treatments within the designated time-period, you will have to pay the regular price for your Botox and will lose all of the additional benefits and the $100 deposit will be lost. There will be NO EXCEPTIONS. **As part of the RMD Botox & Beauty Club benefits, you would receive: 10% off ZO skincare products​ **Cannot be combined with other office specials/offers however you may use Alle Rewards benefits. ***Botox Club will be void: if you no-call/no-show after 120 days from prior treatment, if you decide to do less than 40 units during the allowed time period, if you use Groupon, or if you receive Botox as a Model for training purposes, the $100 buy-in will be lost.
